Prosthetics on implants

Today, one of the most effective methods of dentition restoration is considered to be prosthetic teeth on implants. Unlike clasp or bridge prosthetics, this technology has the greatest physiological effect, does not require dissection of adjacent teeth and allows you to restore functionality for a longer period.

What is implant prosthesis?

Dental prosthetics on implants are the manufacture and installation of various types of artificial teeth (dentures, crowns on implants), which are supported and held by dental implants. Aesthetically and functionally, the implant is no different from a real tooth.

A dental implant is a titanium pin that is surgically implanted in the jawbone under anesthesia to further install an abutment, and a crown instead of a missing tooth in the dentition.

In the process of prosthetics, the dentist uses:

  • gum former – a metal element screwed into the implant to form a gingival ring for further prosthetics;
  • abutment – the adapter head between the implant and the crown;
  • metal-ceramic crown on the implant – an artificial tooth made of biocompatible material, similar to this tooth in terms of aesthetic and masticatory functions, is installed on the implant.

Stages of dental prosthetics types on implants

Conventionally, the price of turnkey prosthetics is formed in 4 stages:

  1. Implant preparation and installation;
  2. Opening the implant and installing the gum former;
  3. Casting and crown making;
  4. Crown fixation.

STAGE 1 – preparation and installation of the implant (how to install the implant?)

Preparation before installing a denture , depending on the state of health of the oral cavity and the body as a whole, takes from a few days to 2 months.

How much does dental implantation cost?

It is possible to tell after when the dentist carries out inspection and diagnosis, finds probable contraindications and estimates the general condition. If necessary, rehabilitates the oral cavity: cleaning, caries treatment, removal of teeth or roots that are not treatable. With the help of X-rays and computed tomography (CT) studies the condition and volume of bone tissue, and then performs calculations on the skin situation and consult with related specialists.

Based on the diagnosis, the doctor decides on the possibility of prosthetics on implants and the features of treatment. Implant placement surgery is performed under general or local anesthesia, including incision of the gums, formation of a bone bed, implant placement, and suturing of the gums.

STAGE 2 – opening the implant and installing the gum former

After the implant is completely fused with the tooth bone (3-6 months), the dentist puts a gum shaper. To do this, he dissects the gingival flap, removes the cap from the dental implant , screws the gum former into the implant and, if necessary, sutures.

Due to the gum shaper, the dentist achieves the correct formation of soft tissues (gum circle) around the crown on the implants . This will allow a tight fit around the abutment, and will prevent the possibility of getting food debris and tartar formation. Which can lead to further re-implantation.

Technically, the mold looks like a metal cylinder in the upper part with a thread with a very small step in the lower part, which is screwed into the implant, achieving a strong and reliable fixation.

The standard time for which the shaper is installed when prosthetic teeth on implants – to create the correct contour of soft tissues (2 to 3 weeks).

In the process of healing the gums around the gum shaper, a dense ash ring is formed with soft mucous tissue, which will later perform an aesthetic function. Reproduction of papillae between crowns on implants .

STAGE 3 – removal of casts and production of crowns on implants

After 2-3 weeks, the orthopedist unscrews the shaper of the gums, screwed in its place, the transfer is removed with the impression material of the cast from the upper and lower jaw. Then install the shaper in place until the orthopedic structure.

Impressions of teeth are sent to the dental technician in the laboratory from which the plaster model of teeth will be made. From the plaster model, when using a scanner and digital programs, crowns, dentures on dental implants are created on the computer. To achieve full anatomical compliance with the individual characteristics of the patient: a tight fit and bite.

Computer simulation allows you to make a crown for prosthetics on implants in the shortest time, while previously traditionally its manufacture and fitting took 3-4 visits to the clinic, until the desired result of accuracy was achieved.

STAGE 4 – fixation of the crown on the implant (photo)

After the crown is made (after 5-10 days), the dentist fixes the crown on implants with cement and screw fixation .

Cement for fixing orthopedic structures (usually used for front teeth) uses a cement-based material for better adhesion of metal and ceramics. This method requires diligence and careful adherence to technology. Timely removal of cement residues so that it does not run under the gums and does not cause inflammation around the crown on the implant (photo).

Screw fixation on the implant (usually used for masticatory teeth) uses the installation of a crown with an abutment on the screw. On top of this crown is a hole through which the crown is attached to the implant with a screw. The hole is then closed with photopolymer material and ground. Later, if necessary, the implant prosthesis can be easily removed without damage.

How to choose crown fixation on implants?

Fixed prosthetics on implants with screw fixation has many serious advantages over cement, but each situation is individual, and the choice of one of them depends on the decision of the orthopedic dentist and the following factors:

  • forecast and possible replacement of structural elements;
  • accessibility to the intervention site;
  • occlusal output parameters;
  • production and cost of different types of restorations on implants;
  • the need to ensure passive landing of the structure.

Crowns on dental implants can be either single or partial dentures on implants or in the complete absence of teeth: removable, conditionally removable and non-removable. Removable ones can be installed in the mouth or removed, and fixed ones are firmly fixed and are the most optimal when it comes to restoring individual teeth.

Benefits of dental prosthetics on implants

Prosthetics on implants in Kyiv has a number of advantages over classic prosthetics (NOT turning of neighboring teeth).

Among the main advantages of prosthetics on implants are:

✓ ease and simplicity of care;
✓ long service life (20-25 years);
✓ high aesthetic effect;
✓ uniform load distribution;
✓ you can install any prostheses;
✓ Neighboring healthy teeth are NOT sawn, and the strength and stability of implants are much higher.

At LYPYNSKY Dental Center, orthopedists carefully study the situation of patients, making optimal plans and methods of dental prosthetics (Kyiv).
